Agreement, waiver, and release

Please fill out the following form.

By participating in our Nordic walking class, I, [Participant Name], acknowledge and agree to the following terms:

1. Assumption of Risk: I understand that Nordic walking is a physical activity that involves certain risks, including but not limited to slips, trips, falls, and injuries. I voluntarily assume these risks and participate at my own risk.

2. Waiver and Release: I hereby waive and release Nordic Walking California and its instructors from any liability, claims, demands, or causes of action arising from any injury, damage, or loss that may occur during or as a result of my participation in the Nordic walking class.

3. Physical Condition: I confirm that I am physically able to participate in the Nordic walking class and have consulted with my physician, if necessary. I understand that it is my responsibility to inform the instructor of any medical conditions, injuries, or limitations before the class begins.

4. Indemnification: I agree to indemnify and hold harmless Nordic Walking California and its instructors from any claims, actions, or losses arising from my participation in the class or any breach of this agreement.

5. Photography and Media Release: I grant permission to Nordic Walking California to use photographs, videos, or other forms of media taken during the class for promotional or marketing purposes.

6. Code of Conduct: I agree to follow all instructions given by the instructor and maintain a respectful and courteous demeanor towards other participants and the instructor.

7. Cancellations and Refunds: I understand that Nordic Walking California has a cancellation and refund policy, and I agree to abide by its terms.

By signing this agreement, I acknowledge that I have read, understood, and agreed to all terms and conditions mentioned above.
